Permission for Youth to Participate in Archery Activities *

I hereby give permission for my above-named child to participate in organized archery activities offered by the International Wildlife Museum for the Archery Day.

It is my understanding that my child will learn, understand and follow established guidelines for safely doing these activities. I further understand that trained instructors will be present to assure that my child knows how to perform the activity safely, and to supervise my child at all times.

Release from Responsibility, Assumption of Risk, and Waiver for Youth Participant *

I agree to the terms below.

I acknowledge that I have been informed of the nature of the activities, and that I am aware of the hazards and risks which may be associated with my above-named child’s participation in the above named activities, including the risks of bodily injury, death or damage to property which may occur from known or unknown causes. I understand, accept, and assume all such hazards and risks, and waive all claims against IWM, SCIF and SCI. I understand that I am solely responsible for any costs arising out of bodily injury or property damage sustained through my child’s participation in normal or unusual acts associated with the above named activities.

Read this document completely before signing. Document must be signed and returned by the day of class in order for you and your child to participate. Its effect is to release the International Wildlife Museum (IWM), Safari Club International Foundation (SCIF) and Safari Club International (SCI) from any liability resulting from you and/or your child’s participation in the activities described below, and to waive all claims for damages or losses against IWM, SCIF and SCI which may arise from such activities.
